For most adults, consuming up to 400 milligrams of caffeine a day appears to be safe for the heart, according to a new scientific statement from the American Heart Association.

That amount is roughly equivalent to three to five 8-ounce cups of regular brewed coffee, but it should be viewed as a general upper limit rather than a daily goal. Many common mugs and coffee-shop drinks are larger than 8 ounces, and caffeine content can vary widely depending on the beans, brewing method and serving size.

The statement, published in the journal Circulation, reviewed existing research on caffeine, coffee and cardiovascular health. It found that moderate coffee consumption was not associated with a higher overall risk of heart disease for most adults. Some studies also linked coffee drinking with lower risks of heart disease, heart failure, stroke and type 2 diabetes.

Those findings do not prove that coffee prevents those conditions. Much of the available research is observational, meaning researchers can identify patterns but cannot determine whether coffee itself caused the apparent benefit.

Coffee also contains many compounds besides caffeine. Some may have antioxidant or anti-inflammatory effects, making it difficult to separate the effects of caffeine from those of coffee as a whole.

“Caffeine consumed in coffee is a key part of daily life for millions of people, and in our review of the most recent research, for most adults, intake of up to 400 mg of caffeine/day, the equivalent of up to 5 cups of caffeinated coffee per day without added sugars or fillers, is safe and does not increase cardiovascular risk,” said Gregory M. Marcus, chair of the statement’s writing group and a professor of medicine at the University of California, San Francisco.

How caffeine affects someone can depend on genetics, age, medications, underlying health conditions, metabolism and habitual caffeine use. Some people may tolerate several cups of coffee without noticeable effects, while others may experience anxiety, disrupted sleep, heart palpitations or a temporary increase in blood pressure after a much smaller amount.

Caffeine generally reaches its highest concentration in the bloodstream within about an hour. In the short term, it can increase alertness, heart rate, blood pressure and blood sugar. Regular consumers may develop some tolerance to these effects over time.

The statement found that coffee’s relationship with blood pressure is not straightforward. In people with optimal blood pressure, drinking one to three cups a day was associated in some studies with a higher risk of developing high blood pressure, while drinking more than three cups was associated with a lower risk.

That does not mean drinking more coffee lowers blood pressure. The findings may reflect differences among coffee drinkers that researchers could not fully measure. High doses of caffeine, particularly from concentrated products such as energy drinks and energy shots, can significantly raise blood pressure, especially in people who already have hypertension.

The evidence on heart rhythm is also mixed. Recent analyses found that drinking one to three cups of caffeinated coffee a day was not associated with a higher risk of atrial fibrillation, a common irregular heart rhythm. Some randomized research has even linked coffee consumption with a lower risk of atrial fibrillation.

At the same time, caffeinated coffee may increase premature ventricular contractions, or PVCs. These are extra heartbeats that begin in the lower chambers of the heart. They are often harmless but may feel like fluttering, pounding or skipped beats.

Very high caffeine intake, including amounts found in some energy drinks and shots, has been linked with abnormal heart rhythms even in otherwise healthy younger adults.

“High doses of caffeine, such as those found in energy drinks including energy shots, may have harmful effects on the heart and should be avoided,” Marcus said.

The source of caffeine matters. Most cardiovascular research has focused on coffee, so its findings cannot automatically be applied to tea, soda, energy drinks, caffeine powders, supplements or medications.

Energy shots may contain three to four times as much caffeine per ounce as regular brewed coffee. Some energy products also contain additional stimulants or ingredients that may affect how quickly caffeine is absorbed or intensify its effects.

Coffee preparation may matter, too. Cafestol, a naturally occurring compound in coffee, has been associated with higher LDL cholesterol in randomized trials. It is more common in unfiltered coffee, including French press, Turkish and boiled coffee. Paper filters remove much of the cafestol, while instant coffee also tends to contain less.

The statement also linked regular black coffee consumption with a lower risk of developing type 2 diabetes, though caffeine itself may temporarily reduce insulin sensitivity. This is another reason researchers caution against assuming that caffeine explains all of coffee’s potential effects.

What people add to coffee can also change the overall nutritional profile. Sweetened coffee drinks may contain substantial amounts of added sugar, while some cream-based drinks can be high in calories and saturated fat. However, the current evidence does not establish that an ordinary amount of milk or cream cancels out coffee’s potential benefits.

The statement does not suggest that people should start drinking coffee for their heart. It also does not mean that 400 milligrams is appropriate for everyone.

People who are pregnant, sensitive to caffeine, taking certain medications or managing high blood pressure, heart rhythm problems, anxiety or sleep disorders may need a lower limit. Anyone who notices palpitations, shakiness, worsening anxiety or disrupted sleep may benefit from reducing caffeine even when their intake falls below 400 milligrams.

“Although research suggests that caffeine consumption may be associated with certain cardiovascular benefits for some people, it’s important to remember that there is no ‘one-size-fits-all’ strategy for safe caffeine consumption,” Marcus said.

The most practical approach is to consider total caffeine intake across all sources, pay attention to serving sizes and notice how the body responds. A traditional 8-ounce cup is considerably smaller than many mugs and commercially sold drinks, so counting cups alone may underestimate actual caffeine intake.

For most adults, moderate coffee consumption appears compatible with heart health. But the dose, source, preparation method and individual response all matter more than a simple cup count.

The scientific statement was prepared by a volunteer writing group for the American Heart Association. The Association reports that more than 85% of its revenue comes from sources other than corporations, including individual donations, foundations, estates, investments and educational materials. Corporations, including pharmaceutical and medical device companies, also provide donations. The Association states that it has policies intended to prevent donors from influencing its scientific content or policy positions.
